Facility Rental Request

Thank you for your interest in renting our facility. Top Level offers 10 week contracts, one session per week, designed to provide consistent, dedicated training time each week.

Please complete the form below to select your session length and secure your spot. A non-refundable deposit is required at the time of registration, and the remaining balance will be due prior to, or at your first, scheduled session.

Once your enrollment is submitted, we will review your request and verify your chosen dates are available. We will then email you a confirmation, and a secure payment link will be provided to make your deposit.

Liability Waivers & Insurance:

We MUST receive signed Liability Waivers for ALL group members, and proof of insurance BEFORE your group will be permitted into the facility. For your conveniece, you can download a copy here to print, sign, and bring with you OR you can complete them and upload with your request below.

Payment Terms for 60 Minute Sessions

Each contract includes one session per week for 10 weeks. A non-refundable 10% deposit is due at signing to secure enrollment:

60 Minute Sessions – $250 deposit due, $2,250 remaining balance

The remaining balance must be paid prior to or at the first scheduled session. After completing this form, you will receive an email with a secure payment link for the deposit amount.

Payment Terms – 90 Minute Sessions

Each contract includes one session per week for 10 weeks. A non-refundable 10% deposit is due at signing to secure enrollment:

90 Minute Sessions – $320 deposit due, $2,880 remaining balance

The remaining balance must be paid prior to or at the first scheduled session. After completing this form, you will receive an email with a secure payment link for the deposit amount.
